Here Toitū Criteria

The target group for the ‘standard’ service are whānau who are:

  • Of working age (18-64); and
  • In receipt of a main benefit with a medical deferral from work obligations due to a health condition or disability; and
  • Enrolled in a participating general practice in the Auckland region
  • Reside in the Auckland region.

The target group for the Responding Early service are whānau who are:

  • Aged between 16-64 years; and
  • Enrolled with a GP in Auckland region; and
  • Live in the Auckland region; and
  • In part or full-time employment; and
  • Not in receipt of a main benefit; and
  • At risk of losing their employment due to their health condition or disability.
